I/we the parent(s)/legal guardian(s) are aware and understand that the SELBRA Office fee of $11.00 per contestant/rodeo will be invoiced on my entry fee invoice.

I/we the parent(s)/legal guardian(s) are aware and understand, that due to the above contestant not being a NLBRA member, the NLBRA insurance (accident/health) will not cover the contestant if involved in any type of accident while competing in SELBRA rodeos/events.
